What are Patient initiated follow ups

Some clinics offer Patient-Initiated Follow-Ups (PIFU), which allow you to request a follow-up appointment when you need one, rather than attending routine appointments.

You can contact your care team if your symptoms change or worsen, giving you more control over your care.

Why PIFU?

PIFU helps by:

  • Giving you control over when you are seen
  • Reducing unnecessary hospital visits
  • Supporting personalised, convenient care
  • Freeing appointments for those who need them most

How PIFU works

If your clinician feels PIFU is suitable, they will:

  • Explain the symptoms to look out for
  • Give you written guidance
  • Tell you how long your PIFU plan lasts
  • During this period, you will not receive routine follow-ups.

If you do not need to be seen within the timeframe, you may be discharged back to your GP (who can re-refer you if needed).

Some long-term conditions may stay under hospital care. Your specialist will advise you.

How to request a PIFU appointment

If you develop the symptoms your clinician has highlighted, you can:

  • Request an appointment through the MyCare Patient Portal, or
  • Use the link in your PIFU welcome text message, or
  • Call the booking team on 020 8725 0007

You may be asked to describe your symptoms and mention any special requirements.

Your clinical team will review your request and arrange a telephone, video or face-to-face appointment if needed.

When not to use PIFU

Do not use PIFU for:

  • New or unrelated symptoms
  • Urgent concerns
  • Emergencies

For urgent help, contact:

  • Your GP
  • NHS 111
  • 999 in an emergency

If you are waiting for a response

You will normally receive a response within 3–14 days, depending on your clinic.
If your symptoms get worse while waiting, please contact your GP or NHS 111.

Why there is a time limit

Your PIFU plan lasts for a set period, based on clinical advice.
When it ends, your care may return to your GP unless further follow-up is needed.
