We’ve introduced a new way to make your appointments even more valuable. For certain visits in Urology and Neurology, you’ll receive a brief online questionnaire to fill out before your appointment. This will help your doctors and nurses understand your needs better and ensure that we can make the most of your appointment.

Here’s what you need to know:

• Where to Find It: Log in to your MyCare Patient Portal Login to MyCare St George’s and click on the “Questionnaire” section.
  • Please check both the “To Complete” and “Health Record” sections. Depending on the information your clinician needs from you, it may appear in different places.

• When to Expect It: Look out for a SMS, appointment letters and check your portal for any questionnaires due for your appointments.
• Why It Helps: Your answers will  give your specialist information to assist them help you as much as possible
