Homelessness Inclusion Team – Service Directory
This service directory was put together by The Homelessness Inclusion Team (HIT) at St George’s Hospital, Tooting. We are a multi-disciplinary team, specialising in homelessness and health and are based in the hospital. The advice and services listed in this booklet are all places where you can access support in the local area once you have left the hospital. Currently Homeless
STREETLINK
If you are rough sleeping you can contact StreetLink via their website: streetlink.org.uk, mobile app or phone line and alert local authorities and street outreach services in England and Wales. If you cannot access the internet you can call: 0300 500 0914 (you may have to hold for a long time) You need to provide details on where you are rough sleeping, street name, park name, a nearby shop, so that an outreach worker will be able to find you. It can be extremely difficult to find people in the dark so detailed information increases your chance of being located. If you have a phone number let them know.
Most outreach teams will go out at night and during the early hours of the morning so stay in that location until you are found, as long as it is safe for you to do so. In most areas of London teams will be going out every night but this is not guaranteed. For any people they are not able to reach that night, they will use the information from the StreetLink alerts on their next shift. You may need to stay several nights in a row before you are found.
If the team find you but are unable to support you that night, they will pass your details on to local outreach teams who can continue to work with you. If this is your first contact with Streetlink, they may refer you directly to a service that will start the process of finding you accommodation. You will only be eligible to statutory help with accommodation if your immigration status enables you to access public funds that entitle you to homelessness assistance. If you are not entitled to access public funds there are a few places listed in the NO RECOURCE section of this directory who may be able to provide you with specific advice and assistance.
SUPPORT FROM YOUR LOCAL COUNCIL
If you are entitled to statutory support, are rough sleeping, sofa surfing or at risk of losing your accommodation in the next 2 months you can approach your local council for support. You can also be considered legally homeless if you are unable to return home if you are experiencing domestic abuse or fleeing/at risk of serious violence. If you attend a day centre or have contact with an outreach team they may be able to assist with this.

GETTING A GP (DOCTOR)
- Anyone in UK can register with a GP.
- You do not need a fixed address (but they may have a catchment area)
- You do not need identification
- Your immigration status does not matter

GETTING A DENTIST
Registering with a dentist is not as restrictive as a GP. You do not need to keep to your local area. Just find a surgery that is convenient to you and ask if they have any spaces.

If after contacting several surgeries you can’t find one accepting NHS patients, call the contact centre on: 0300 311 2233
You do not need to pay for NHS dental Services if you are:
- Under 18 (or 19 and in full time education)
- Pregnant or had a baby in the last 12 months
- Being treated in hospital
- On benefits (you or your spouse)
- Have a valid HC2 Certificate
(You will need to provide proof of the above circumstances to the surgery)
Call 111 if you have any of the following:

- Bleeding from your mouth
- Or Trauma (broken tooth) They can assist with an appointment with an emergency dentist.
GUY’S HOSPITAL EMERGENCY DENTAL CLINIC

They accept walk-in appointments for dental emergencies from 8am Mon-Fri, but expect a long wait. It is easier if you can get 111 to make a referral.

This centre offers a comprehensive sexual health and contraception service and manages complex sexual health issues, led by nurses and doctors.

THE RAINBOW HEALTH CENTRE
