Discharges

Where possible, discharges will be planned in advance, the patient informed and discharge will be in the morning. The patient will be given any prescriptions they need to take with them (their TTO’s) and a discharge letter. A copy of the discharge letter will be sent direct to you as their GP.

If the patient’s medical and or social circumstances have changed since admission their requirements will be managed by a multi-disciplinary team.
