Community palliative care team

The community palliative care team are usually based at a hospice, however this could differ depending on your location.
They are made up of different healthcare professionals such as; specialist nurses, specialist doctors, physiotherapists, occupational therapists and social workers however this can differ between hospices.
The community palliative care team are an excellent source of support and can help with; symptom control, emotional/psychological support, social/financial support, end of life care, advance care planning, carer support and palliative rehabilitation. There may be more services available depending on which hospice you are linked to.
Your local hospice or the community palliative care team will depend on where you live and where your GP is, if you are unsure then please ask your healthcare professional for guidance.
A referral to the the community palliative care team can be made by your healthcare professional.
