Making, changing or cancelling your appointment

Change or cancel your appointment

You can change or cancel your appointment, and update your contact details by completing the form below. You can also use this form if you were asked to contact us following a missed appointment.

Do not fill in this form if:

  • Your appointment is within the next 24 hours – Please call us immediately at 020 8725 0007.
  • Your appointment is with Moorfields Eye Clinic – To cancel or reschedule, call 020 8266 6334.
  • Your appointment is with Diagnostics and Imaging (X-ray or Scans) – Please call 020 8725 3037.
  • Your query relates to Patient Transport – Please call the team on 020 8725 0808.

What happens next:

  • If you are discharged you and your GP will receive a discharge letter confirming this.
  • If you requested to cancel and rebook, we will aim to rebook your appointment if an appropriate slot is available. If not, your request will be passed to the relevant team to find a booking solution. You will receive a confirmation letter once this is processed. Please note if you cancel more than twice you may be discharged.
  • If you submitted an update to your details, we will update our records.

We will not contact you via telephone unless we need further information or if your request cannot be processed.
