From March 31, 2025, we’re introducing a new way to help make your appointments even more valuable. For some appointments in Urology and Neurology, you’ll receive a short online questionnaire to complete before your visit. This helps us understand your needs better and prepare for a more helpful conversation with your care team. It is important that you complete the questionnaire before your appointment.

Here’s what you need to know:

• Where to Find It: Log in to your MyCare Patient Portal and click on the “Questionnaire” section. Please check both the “To Complete” and “Health Record” sections. Depending on the information your clinician needs from you, it may appear in different places.

• When to Expect It: Look out for a SMS, appointment letters and check your portal for any questionnaires due for your appointments.
• Why It Helps: Your answers will  give your specialist information to assist them help you as much as possible
