Day Hospital offers an interim facility between acute and primary care settings for people over 65 years, resident in London Borough of Wandsworth who need at least 2 disciplines’ intervention. Referral can be made by a health professional from SGH and other NHS Trusts, CSW and other PCTs using the Day Hospital referral form.
Day Hospital offers multidisciplinary assessment/intervention.
Service users attend for half-day sessions twice a week for a period of weeks and transport is available.

Treatment/rehabilitation by therapists on individual and group basis
Home visits for assessment or review
Access to nursing review and continence support
Information re health promotion, resources and falls prevention Referral to and liaison with other services and teams as part of comprehensive intervention and discharge planning

Specialist Registrar and ST1 access with weekly consultant-led review meeting
Band 7 occupational therapist/team co-ordinator
Band 6 nurse and band 2 HCA
Band 7 and band 5 physiotherapist plus 2 x band 2 therapy technicians
