What is Complex Case Management?
Complex Case Management provides care and support for people with complex needs where a systematic and coordinated approach to their care is required, through a process of assessment, planning, and delivery 365 days per year.
The four Complex Case Management Locality Hubs are:
- East Wandsworth
- North Wandsworth
- West Wandsworth
- South Wandsworth
A reactive response to urgent requests for community services input to enable patients to be managed in an acute phase at home thereby avoiding admission to hospital and assistance in the safe, early discharge of patients from secondary care back in to the community where difficulties are encountered via usual pathways
The Complex Case Management Team provides proactive management of patients in the community with long term/chronic conditions to reduce the number of unplanned admissions to secondary care.
Operating hours: Monday to Friday 09:00 to 17:00
Who can refer to Complex Care Management?
Patients, and with their consent their families, carers, Healthcare Professionals, Social Care Providers and Voluntary Services are able to refer in to services via Access and coordination
