Working together with Wandsworth Clinical Commissioning Group we have redesigned our community adult nursing and therapy services to adapt to different healthcare requirements, ensuring the best response to individual patient needs. The table below shows what the new services are now called. If you click on the button you can find out more information about each service.

We provide accessible services with the aim to prevent admissions to hospital and facilitate and support safe discharge home. We also support the management of patients with long term conditions, patients with on-going health needs and patients who need support to regain and/or maintain their independence.

All referrals to CAHS are triaged (assessed/checked) by our Access and Coordination team to ensure that patients receive the most appropriate support for their healthcare needs.
