Introduction/service description

Community Nutrition & dietetics

  • Provides nutritional management for Wandsworth residents of all ages referred with a variety of clinical conditions; to improve outcomes, promote health and prevent further complications.
  • Prevent or reduce the incidence of nutrition related ill health in Wandsworth, aiming to reduce/avoid hospital admissions.
  • Provide a specialist resource/training for other community health professionals in the area of clinical nutrition and health.

Staff/team profile

  • The community dietetics service consists of 16 staff, covering both adults and children who have a Wandsworth GP.
  • The service is split across two sites: one team is based at St John’s Therapy Centre and the other team is based at Queen Mary’s Hospital.

Eligibility for service

  • Any child or adult with a Wandworth GP can be referred to the Community Dietetics Service who meets the referral criteria (see referral forms).
REASON FOR REFERRAL CRITERIA
DIABETES
  • Type 2 diabetes (include blood results confirming diagnosis e.g. HbA1c from last 3-6 months), impaired glucose regulation or non-diabetic hyperglycaemia.

  • Type 1 diabetes should be referred to Beta Cell, QMH.
WEIGHT MANAGEMENT • BMI ≥ 25 or self refer to Commercial Voucher scheme on 020 8812 6700 (Slimming World or Weight Watchers)

CARDIOVASCULAR DISEASE •Primary or secondary prevention – include recent lipid profile /BP
NUTRITION SUPPORT • Malnutrition/Underweight – identified as high risk by a MUST (Malnutrition Universal Screening tool) score of ≥ 2 (excluding anorexia or bulimia). For Must scores <2 please refer to www.malnutritionpathway.co.uk

• Swallowing / feeding difficulties after Speech and Language Therapy (SLT)assessment  – SLT report to be attached

• Enteral Feeding eg PEG

• Review of prescribed Oral Nutritional Supplements (ONS)

• Pressure sores

GASTROINTESTINAL DISORDERS / FOOD

INTOLERANCES

• Inflammatory Bowel Disease (IBD) eg, Crohns Disease, Ulcerative Colitis

• Coeliac Disease (diagnosed by biopsy) – results to be attached and any recent blood tests

• Irritable Bowel Syndrome  -blood results to be attached confirming negative Coeliac screen and inflammatory bowel disease

• Food intolerance

NUTRITIONAL

DEFICIENCIES

• Anaemia –  blood results to be attached

• Other nutrient deficiencies eg Vitamin D, calcium – blood results to be attached

EATING DISORDERS

We do not accept referrals for people with eating disorders.

Refer to Specialist Eating Disorders Service, Psychological Therapies or Community Mental Health Team.

  • Community Dietetics clinics in 3 sites across Wandsworth : St John’s Therapy Centre, Queen Mary’s Hospital and Tooting Health Centre ( adults and children)
  • Domiciliary service for housebound adults /children with complex special needs
  • QMH inpatients (Mary Seacole and Gwynn Holford)
  • Day hospitals at St John’s Therapy Centre and Queen Mary’s Hospital
  • Wandsworth Care homes and Ronald Gibson House  In Reach beds
  • Home Enteral Tube Fed (HETF) patients for adults and children
  • Beta Cell Specialist Diabetes outpatients clinics at Queen Mary’s Hospital
  • Diabetes education groups for type 1 and type 2 patients
  • Group education sessions: 6 week Weight Management course, Low FODMAP, Bone Health/Falls, COPD, Cardiac Rehab, Stroke, Fatigue
  • MDT feeding service for children with behavioural feeding problems
  • Special schools (Linden Lodge,  Paddock, Greenmead)
  • Prescribing advice/audit of Oral Nutrition Supplements in primary care
  • Training, support and advice to other HCPs.

Eating Disorders: 

Please note that both our community and hospital dietetic services do not accept referrals for eating disorders. Referrals for eating disorders should be addressed to Springfield Hospital if you require a Dietitian for this purpose.

Opening hours

Community Dietetics Service operates a 9-5pm Monday-Friday service with some early morning/evening clinics /group sessions.

There is no community dietetic weekend service available for adults or paediatrics.
