Discharge from hospital

Where possible, discharges will be planned in advance, the patient informed and discharge will be in the morning. The patient will be given any prescriptions they need to take with them (their TTO's) and a discharge letter. A copy of the discharge letter will be sent direct to you as their GP.

If the patient's medical and or social circumstances have changed since admission their requirements will be managed by a multi-disciplinary team.

The Trust discharge team gives advice and support to all clinical areas and can be contacted on:

Tel: 0208 672 1255 bleep 6568.

On some of the wards there is a dedicated discharge nurse. For geriatric medicine the discharge co-ordinator can be contacted by ringing:

Tel: 0208 672 1255 bleep 7358.
